Physical Exams

All athletes must have the NCHSAA physical examination form completed to participate in athletics during the school year. This must be completed by a physician and uploaded to the DragonFly system.

Free physical exam screenings provided by the NHC Health Department once per year. The 2024 date of these screenings is Thursday, May 30, 2024. Athletes must bring the NCHSAA examination form with them to the screening. 

Vaccinations will also be given again this year. Athletes must bring the completed vaccination permission slip with them to the screening. 

The health department will NOT have copies of either form available on the day of the screenings. 

*Copies of completed PPE’s are not provided for athletes to take home. Please bring a cell phone to take a photo of your completed physical.*
